2003, 2005, 2007 & 2010 Nesson Award for Excellence for APP’s Asthma Disease Management, Patient Experience, Obesity Management Programs & Driving Performance by our Website Design Project
Established in 2000, the prestigious Nesson Award is presented in honor of H. Richard Nesson, MD, the founding father of Partners Community HealthCare, Inc. (PCHI), who conceived the idea of PCHI as a means to integrate and manage patient care. The awards recognize programs that exemplify PCHI's best practices and its mission to enhance both the physician-patient relationship and the physician experience through innovation in medical practice and collaboration among physicians and medical centers in pursuit of excellence. Awards were presented in three categories – collaboration, innovation and excellence.
Affiliated Pediatric Practices has received the award in 2003, 2005, 2007 & 2009. APP was first celebrated in 2003 for its excellence in their highly successful and innovative Asthma Disease Management Program. In 2005, Affiliated Pediatrics was honored to receive the prestigious "PCHI Nesson Award for Excellence" - for implementation of our Patient Experience Improvement Program. In 2007 APP won for their Obesity Management Program and again in 2010 for Driving Performance by our Website Design Project.
2002, 2003, 2004, 2005, 2006, 2007, 2008 and 2009 Harvard Pilgrim Healthcare Physician Group Honor Roll In recognition of Excellence in Preventive and Chronic Illness Care for Pediatric Patients.
Affiliated Pediatric Practices was named to this Honor Roll which recognizes physician groups that have achieved exceptionally high scores for measures of clinical quality encompassing both disease prevention and the treatment of chronic illness. Harvard Pilgrim selected physician groups for its Honor Roll based on clinical performance against national HEDIS quality benchmarks.
HPHC identified practices whose performance exceeded the national 90th percentile on a majority of performance scores as reported by the National Committee for Quality Assurance.
2001, 2002, 2003, & 2004 Tufts Health Plan Certificate of Excellence for the Physician Group Quality Profile
The Tufts Health Plan Certificate of Excellence was given to Affiliated Pediatric Practices which received a rating of “Excellent” in serving its pediatric patients.
Massachusetts Healthcare Quality Partners - Statewide Comparative Clinical Quality 2009 Reports
Affiliated Pediatric Practices received the highest rating of the MHQP annual Clinical Quality Report (CQR). The 2009 SCCQR provides 2008 performance results and includes 2008 state and national benchmarks in healthcare quality. APP exceeded the 90th percentile in several of the pediatric categories in preventive and chronic disease services. (http://www.mhqp.org )
